Please find below the Spring 2009 Registration Flyer. Please distribute to individuals that would like to register for Spring soccer.
Medical Release
Soccer is a very safe sport. But, as with any sport, accidents can happen. If they do, TAFC wants to be able to have players given immediate medical treatment and notify parents. All players affiliated with the club must have a signed, up-to-date Medical Release on file with the club, and a copy with the Coach.
Get the Medical Release form here. Fill it out and turn it in to the Coach as soon as possible, but before the first game of each season.
